What They're Looking For.
Must Have
Associate's Degree or Baccalaureate School of Nursing Degree, Current and valid license as a Registered Nurse in the state of Indiana, 2 year of experience as a Registered Nurse in a clinical setting
Nice to Have
1 year of home health experience, Case management/supervisor experience, Pediatric/adult care experience, Knowledge of Medicaid paid family caregiving program, Knowledge of HHA scope of practice, Knowledge of OASIS
What You'll Do.
Manage a diverse caseload
ensuring quality patient care and efficient care coordination
and support to Home Health Aides (HHAs)
Develop and implement individualized care plans based on patient assessments and goals
and internal support teams
Provide education and support to patients and families
Maintain accurate and timely electronic charting
Ensure all documentation meets regulatory and Abby Care standards
Establish and maintain effective communication with PCPs
and internal support teams
Escalate appropriate issues to Clinical Leads
Participate in on-call rotation
Achieve caregiver satisfaction
Monitor and work to improve patient readmission rates
Ensure extended visits remain within acceptable ranges
Complete administrative tasks thoroughly

This role will report into the Director of Nursing in Indiana. This is a Full-Time opportunity based in the Southwest Indiana Area, specifically Mooresville, Martinsville, Plainfield. The RN Case Manager at Abby Care provides the comprehensive care and coordination of services for patients within their caseload. This role requires a nurse with excellent organizational skills, strong communication abilities, and a dedication to providing high-quality patient care.
