Must Have
High School Diploma, At least 18 years of age, Valid Driver's License, 2 years paid experience, 1 year working directly with Tailored Care Management eligible population, Performing general outreach, Coordinating services/appointments, Engaging in health promotion activities, Sharing information with care manager, Providing and tracking referrals, Participating in case conferences, Support care manager in assessing unmet needs, Lived experience with I/DD or TBI, Demonstrated knowledge of NC Medicaid delivery system, Lived experience with behavioral health condition, Certified Peer Support Specialist, Parent or guardian of individual with I/DD or TBI or behavioral health condition, 2 years direct experience providing care for individual, Navigating Medicaid delivery system on behalf of individual

What You'll Do.
Coordinate services and appointments
