Must Have

Master's degree in human service field, 1 year experience MH/SU/IDD, Bachelor's degree in human service field, 2 years experience MH/SU/IDD, Bachelor's degree non-human service, 4 years experience MH/SU/IDD, 2 years LTSS/HCBS coordination, Care coordination experience, Care management experience, Case management experience, Valid driver's license

Nice to Have

Clinical license preferred

What You'll Do.

Foster and facilitate care management process

Conduct Care Management Comprehensive Assessments

Share information and facilitate Tailored Care Management Planning

Monitor ongoing care management plan implementation

Facilitate Care Plan Updates

Stay up to date with best practices
