Ensures sustained high quality of care and services to persons with mental health, substance use and/or developmental disabilities. Complies with company, state and federal policy. This is a hybrid position. It is a community-based position, and it does require travel within and surrounding the county. It offers some flexibility. in the structure of the schedule. _**Required Education/Experience**_ **Education** \- For all populations served, Care Managers must meet at least ONE of the following qualifications to meet the criteria as a Qualified Professional (QP), per 10A NCAC 27 .0104 * A master’s degree in a human service field** AND at least one year of full-time experience working with the MH/SU/IDD population* * A bachelor’s degree in a human service field** AND at least two years of full-time experience working with the MH/SU/IDD population* * A bachelor’s degree in a non-human service field AND at least four years of full-time experience working with the MH/SU/IDD population* **Experience** - * For care managers serving members with LTSS needs: Must meet at least one of the qualifications to meet QP criteria, as stated above AND Two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience.
