What They're Looking For.
Must Have
High school diploma/GED and at least one year of paid or volunteer experience working with adults with mental illness and/or substance abuse (for candidates with lived experience with substance use disorder who have 2 years sustained sobriety)
Nice to Have
Bachelor’s degree or Master’s degree in a human services field or social work or related field
What You'll Do.
Empower residents with exercising autonomy and independence in all aspects of their life and assists with identifying choices and developing skills in self-advocacy.
Conduct initial needs assessment and collaboratively develop a plan with the resident to obtain benefits/entitlements and other community-based resources.
Support new residents in all phases of transitioning into housing in collaboration with partnering organizations.
Assist clients in being fully integrated into their communities by supporting them with resource utilization and/or obtaining memberships and natural supports.
Provide practical help and mentoring/modeling
side-by-side individualized support
problem solving and direct assistance to help clients obtain skills needed for daily independent living.
Promote concepts of health and wellness by assisting clients with accessing information and services related to medical care including HIV care resources
and exercise as appropriate.
Intervene in potential crisis situations
utilizing trauma informed
verbal de-escalation techniques.

Collaborate across the staff team to ensure that resident perspective is always incorporated, that preferences are recognized and that choices are maximized.; Support new residents in all phases of transitioning into housing in collaboration with partnering organizations.
