We are so glad you are interested in joining Sutter Health! **Organization:** SCAH-Sutter Care at Home - Valley **Position Overview:** This position facilitates admissions services by providing inpatient supportive care and seamless transition to home health/hospice. Assists the organization by assuring that the right care is provided at the right place and the right time. Positively promotes Sutter as the home health/hospice provider of choice within the communities for home health/hospice services. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, Durable Medical Equipment (DME). This position enters referral into electronic medical record and completes referral process and admits patient. Serves as a resource by supporting the mission and vision of the organization. **Job Description** : **EDUCATION:** * Other: Graduate from an accredited school of nursing **CERTIFICATION & LICENSURE:** * RN-Registered Nurse of California * BLS-Basic Life Support Healthcare Provider * AUTO-Automobile Insurance * DL-Valid Drivers License **TYPICAL EXPERIENCE:** * **5 years recent relevant experience** **SKILLS AND KNOWLEDGE:** * Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice/home health and palliative care for terminally ill patient. Should also possess knowledge of hospice/home health reimbursement methods. * Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care. * Ability to recognize the special needs of hospice/home health patients and others in the home. * Ability to communicate with patients and significant others in an effective, mature, caring manner. * Ability to maintain harmonious constructive relationships with internal and external customers. * Demonstrated effective written communication skills. * Ability to be aware of surroun
