What You'll Achieve.
ensuring timely assessments and continuity of care for hospice patients and families; delivering dignified, responsive care during critical moments
Industry & Context.
Ability to lift 50 pounds, Must be comfortable working in environments with animals and pets, Flexibility to work outside regular hours based on business needs, PM shift, Friday - Tuesday, 3:30 PM - 12:00 AM, background check, drug screening, physical, TB testing (if applicable), E-Verify to confirm work authorization
What They're Looking For.
Must Have
Registered Nurse with a valid state license, Minimum of 1 year of nursing experience required, Valid driver’s license, good driving record, reliable transportation, Must clear a criminal background check, physical, drug screening
Nice to Have
Hospice experience preferred
What You'll Do.
Perform comprehensive nursing evaluations and after-hours assessments for patients with changes in condition
Assess for signs and symptoms of discomfort
Initiate and update plans of care as provide nursing follow-up and caregiver instruction
Submit accurate documentation within 24 hours of visits
Coordinate services across settings to ensure quality and appropriateness of care
Follow approved care plans
and communicate with physicians when appropriate
