Purpose: Ennoble Care is hiring for a full-time Hospice RN Case Manager who resides within or in close proximity to Chester County, PA. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers. What You'll Do: Manage your assigned patient caseload within your territory and be responsible for the following tasks, including but not limited to: assessing, planning, implementing, and evaluating of patients care throughout all phases of the nursing process. Document all notes in the EMR and submit them accurately and in a timely manner Communicate and partner cross-departmentally with all attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care for all patients Educate and coach all relevant parties on the best practices regarding call levels of care for the patient including: physical, mental, emotional, etc Partner with clinical leadership to manage/address any issues, share professional support, and exchange feedback to enhance professional growth
