What They're Looking For.
Must Have
Valid RN license in Pennsylvania, 1-2+ years experience as RN, 1+ year hospice care experience, Valid driver's license, Reliable vehicle
Nice to Have
Hospice care experience preferred
What You'll Do.
Manage assigned patient caseload
Implement patient care
Evaluate patient care
Document all notes in EMR
Submit EMR notes accurately
Communicate with physicians
Partner with hospice staff
Coordinate patient care
Educate parties on best practices
Coach parties on best practices
Manage issues with leadership
Address issues with leadership
Share professional support
Attend patient care conferences
Facilitate discussion of issues
Document NAC supervision
Respond to patient needs
Provide support at time of death
Provide support during bereavement
Participate in Hospice orientation
Participate in in-service training
Participate in community programs
Maintain familiarity with agency policies
Maintain familiarity with State rules
Maintain familiarity with Federal rules
Accept other assignments
