What They're Looking For.
Must Have
Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education, Baccalaureate degree in psychology, sociology, or other fields related to social work and 1 year of social work experience in a healthcare setting, Minimum of one year's experience in health care, Possesses and maintains current CPR Certification, Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order, Able to pass a background check and has current fingerprint card clarence
Nice to Have
Hospice experience preferred, Experience in a hospice care preferred
What You'll Do.
Assess psychosocial status of patients and families
Communicate findings to registered nurse and interdisciplinary group
Provide assessment in patient's residence
Assist in planning for future needs/placement
Carry out social evaluations
Plan interventions based on evaluation findings
Counsel patient and family/caregivers
Maintain clinical records on all patients
Provide information and referral services
Coordinate care with community agencies
Remain available for urgent needs
Participate in individualized plan of care development
Attend interdisciplinary group meetings
Assist physician in understanding social and emotional factors
Participate in quality assessment performance improvement
Engage patient in discharge planning needs
How You'll Work.
Team & Collaboration
Maintains collaborative relationships with organization personnel; Attends regularly scheduled interdisciplinary group meetings
