Must Have
Minimum 2 years of experience in a healthcare setting, Valid driver's license and reliable transportation

Develop and maintain relationships with referral sources
Educate potential patients and families about hospice services
Assess patient eligibility for hospice care
Facilitate the admission process for new patients
Collaborate with clinical teams to ensure smooth transitions
Track and report on referral and admission metrics
