Nice to Have
Minimum of one-year experience in Medicare-certified home health/hospice agency, Demonstrated knowledge of home health nursing/hospice, Demonstrated knowledge of outpatient services
What You'll Do.
Conduct medical evaluations
Order laboratory tests
Perform laboratory tests
Interpret laboratory tests
Order radiology tests
Perform radiology tests
Interpret radiology tests
Prescribe medications
Prescribe controlled substances
Order durable medical equipment
Perform therapeutic measures
Assist patients in identification of goals
Assist families in identification of goals
Communicate goals among Interdisciplinary Team
Consult with palliative care physician
Inform primary physician of services
Collaborate with other physicians
Assist in care coordination
Prepare patient records
Maintain patient records
Ensure continuity of care
Serve as liaison between patient and team
Serve as liaison between patient and specialty areas
Participate in Quality Assurance activities
Participate in committee activities
Comply with applicable laws
Comply with applicable regulations
Provide training to staff
Provide education to staff
Provide onboarding support to staff
Define goals for professional growth
Participate in professional activities
Participate in professional organizations
