What They're Looking For.

Must Have

High School Diploma or equivalent, Completion of a Hospice Orientation within 3 days of hire, At least one year of nursing Assistant or Home Health Aide experience, HHA Certified or Ability to meet one of the following eligibility requirements for Competence: Currently certified CNA. Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program Home health aide with documented home health aide training and competency evaluation who has not been employed as a home health aide for 24 consecutive months, An interest and sensitivity in caring for the terminally ill, Ability to read, writes, understands and carries out directions/instructions and maintains simple records, A pre-employment drug test and physical examination required and an annual health assessment (as required by NYS Health Department) are musts, Must respect patient confidentiality, Accomplishes duties of the job within allotted time, Work is thorough and neat

Nice to Have

Two years of Hospice Aide experience, Participates in the training of new agency home health aides (CHHA) during their initial training/certification, Must be capable of working independently and under minimal supervision, Provides mentoring to new Hildebrandt Hospice Aides as requested, Regularly completes on going hospice education through an on line portal, Completes Safe Food Handling Certification Level ll after 6 months in the role, Demonstrates a higher level of commitment to outstanding customer service both internally and externally, Supervisor recommendation is required, Must be and remain in good standing with no disciplinary action and maintain positive performance measures, Ability to interact, perceive and anticipate the needs of the patient

What You'll Do.

Follows Hospice Plan of Care

Observes patients' conditions

Reports changes in patient condition

Turns and repositions bedridden patients

Serves and collects meal trays

Measures and records food and liquid intake/output

Provides patients assistance with walking

Provides assistance with personal care

Answers patient call lights

Maintains a neat and clean patient area

Completes light housekeeping

Stocks/stores medical supplies

Offers support and comfort to families

Provides post-mortem care

Assists RN and other disciplines

Maintains high standards of integrity

Maintains member privacy

Performs other functions as assigned

Assists RN and other disciplines; Provides mentoring/support to other Home Health Aide’s; Provides hands on experience to new Home Health Aide’s; Works with patients and their respective caregivers; Provides mentoring to new Hildebrandt Hospice Aides
