Provides home visitation services
Provides culturally sensitive home visitation services
Links families to a medical provider
Administers developmental assessment tools and screenings
Works with families to develop individualized family service plans
Maintains accurate and complete electronic
Facilitates relationships and coordinates service
Participates in supervision
Participates in required trainings

Facilitates relationships and coordinates service with program collaboration partners and other community agencies; Ability to work well with and relate effectively with co-workers, staff, or other service providers, parents, young children, and community groups
