By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life. You will report to the Clinical Manager. **We are offering a $10,000 Sign On Bonus for this opportunity!** As a Home Health Physical Therapist, you will: * Confer with physician and clinical team members to obtain patient information and help develop, implement and revise the therapy treatment program. * Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function * Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff * Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plan that reflects current plan of care as related to therapy * Oversee Physical Therapy Assistant and Home Health Aide staff performance, including monitoring the appropriate completion of documentation * Report patient progress to the patient's physician, Clinical Manager, staff, patient and family # **Use your skills to make an impact** **Required Qualifications** * Current and unrestricted Physical Therapy license in the state of North Carolina * A valid driver's license, auto insurance, and reliable transportation * Current CPR certification **Preferred Qualifications** * Home Health experience Pay Range $59.00 - $83.00 pay per visit/unit $93,000 - $128,000 per year base pay **Additional Information** **Driving Statement** You will be a part of Humana's driver safety program and therefore will need a valid state driver's license
