What They're Looking For.
Must Have
Master's or doctoral degree, Current state license as Physical Therapist, 1 year of experience as licensed PT, Reliable transportation, Valid automobile insurance, Willingness to travel daily

Home health experience, OASIS certification, Willingness to obtain OASIS certification, Experience with Medicare documentation
What You'll Do.
Provide comprehensive rehabilitation services
Conduct thorough assessments
Evaluate home environments
Develop individualized treatment plans
Implement physical therapy treatments
Minimize residual physical disabilities
Work toward optimal patient outcomes
Perform OASIS assessments
Collaborate with physicians
Revise comprehensive care plans
Assess ongoing rehabilitation needs
Assess equipment needs
Identify referrals to other disciplines
Instruct on prosthetic/orthotic use
Promote disease prevention
Promote injury management
Work toward patient independence
Provide education on fall prevention
Provide education on safety modifications
Coordinate with healthcare professionals
Attend patient care conferences
Initiate patient care conferences
Communicate patient progress
Communicate condition changes
Supervise physical therapy assistants
Supervise physical therapy aides
Participate in quality improvement
Participate in QAPI activities
Complete accurate documentation
