What They're Looking For.
Must Have
Master's or doctoral degree in Physical Therapy, Current state license as Physical Therapist, 1 year experience as licensed Physical Therapist, Reliable transportation, Valid automobile insurance, Willingness to travel daily for home visits
Nice to Have
Home health experience, OASIS certification, Willingness to obtain OASIS certification, Experience with Medicare home health documentation
What You'll Do.
Provide comprehensive rehabilitation services
Conduct assessments of functional level
Evaluate home environments
Develop individualized treatment plans
Use physical therapy treatments
Minimize residual physical disabilities
Perform OASIS assessments
Collaborate with physicians and care teams
Develop and revise care plans
Assess ongoing rehabilitation needs
Identify referral needs
and caregivers techniques
Instruct on prosthetic and orthotic devices
Promote disease prevention strategies
Promote injury management strategies
Work toward patient independence
Provide education on fall prevention
Provide education on home safety
Coordinate with physicians
Attend patient care conferences
Communicate patient progress
Supervise physical therapy assistants
Supervise physical therapy aides
Participate in quality improvement initiatives
Submit start of care paperwork
Monitor and report patient condition changes
Stay current with physical therapy trends
Participate in discharge planning
