Must Have

Michigan HHA certification, 1+ years of experience, CPRLS certification, Valid driver's license, Reliable transportation, Ability to lift, transfer, and provide hands-on care safely, Comfortable working independently in patients' homes, High school diploma or GED

Previous home health experience, Familiarity with Homecare Homebase (HCHB) or similar EMR, CNA or other clinical training/certification, Experience with medically complex or high-acuity patients, Bilingual (English/Spanish)

- Home Health Aide (HHA) - Comp: $17.00 /per hour + mileage reimbursement - Schedule: Full-time Monday–Friday + some weekends (as needed) - Location: Field-based WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. The HHA role is essential to that mission: you provide compassionate personal care in the home, support patients in their daily activities, and help us raise the standard for what home health can be. OBJECTIVES 1. Carry a full, high-quality caseload 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
