THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. The HHA role is essential to that mission: you provide compassionate personal care in the home, support patients in their daily activities, and help us raise the standard for what home health can be. OBJECTIVES 1. Carry a full, high-quality caseload 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Provide personal care assistance including bathing, dressing, grooming, and toileting. - Assist patients with safe transfers, ambulation, and mobility using proper body mechanics. - Support patients with medication reminders as directed by the care plan. - Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse. - Perform light housekeeping tasks related to patient safety and care (la
