Michigan HHA certification, 1+ years of experience, CPRLS certification, Valid driver's license, Reliable transportation, Ability to lift safely, Comfortable working independently, High school diploma or GED

Previous home health experience, Homecare Homebase (HCHB) familiarity, CNA or other clinical training, Experience with medically complex patients, Bilingual (English/Spanish)

- Home Health Aide (HHA) - Comp: $17.00 /per hour + mileage reimbursement - Schedule: Full-time Monday–Friday + some weekends (as needed) - Location: Field-based WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. The HHA role is essential to that mission: you provide compassionate personal care in the home, support patients in their daily activities, and help us raise the standard for what home health can be. OBJECTIVES 1. Carry a full, high-quality caseload 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Provide personal care assistance including bathing, dressing, grooming, and toileting. - Assist patients with safe transfers, ambulation, and mobility using proper body mechanics. - Support patients with medication reminders as directed by the care plan. - Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse. - Perform light housekeeping tasks related to patient safety and care (la
