Must Have
Qualified Mental Health Professional Status, Peer Support certification, Masters or Bachelor's degree in human services field, 1-2 years experience working directly with adults with MH and SU needs, High School Diploma or GED, Certified as a Peer Support Specialist, minimum of 18 months in recovery, 1 year experience in health care or crisis setting, Complete training specific to crisis response, Valid drivers license, automotive insurance, Pass background check, Pass drug screen

**We are hiring for:** FRTC Discharge Planner **Type:** Regular If you are a positive and personable individual looking for a satisfying and fun opportunity to make a real difference in the lives of people with intellectual, developmental disabilities, and people facing mental health, and substance use challenges, join our team at RHA Health Services! Discharge Planner/CPSS needed for Facility Based Crisis Unit. Serve as primary point of contact for individuals seeking substance use detox and mental health crisis services to coordinate and refer to desired aftercare/discharge plans; works collaboratively with treatment team to locate needed referrals and resources. Provide essential expertise and consultation to the entire team to promote a culture in which each individual’s point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation , and community self-help activities. This position serves as an advocate for the person supported. Performs a wide range of tasks to assist the people supported in regaining control over their own recovery processes. These interventions are strength-based and focused on promoting recovery, symptom reduction, increased coping skills, and achievement of the highest level of functioning in the community. Functions as a role model of competency in recovery and ongoing coping skills. Staff ratio takes into consideration evening and weekend hours, needs of special populations and geographical areas to be served.
