What They're Looking For.
Must Have
Colorado State Licensed Practical Nurse license, 1-year experience in a long-term care facility, 1 year experience using electronic medical record platforms, Must be able to read, write and speak the English language, Must possess excellent customer service skills, computer word processing and database experience
Nice to Have
coordinating care in a clinical environment, caring for residents with dementia a plus, IV Certification helpful
What You'll Do.
Provide clinical services
Coordinate care and services
Perform duties with expertise
Attend Resident Care meetings
Maintain Resident Care Plans
Recognize and identify signs and symptoms
Use initiative and proper judgment
Coordinate interventions
Communicate with nursing staff
Communicate with physician
Communicate with pharmacist
Communicate with resident

Ensure protection of resident privacy
Ensure protection of resident confidentiality
Ensure documentation is done via electronic medical record
Ensure documentation is accurate
Administer medications
Ensure needed medications are available
Destroy expired medications per protocol
Communicate incidents
Communicate progress of residents
Document assessment findings
Document interventions
Provide general oversight
Coordinate services during shift
Report concerns to Director of Nursing
Report concerns to supervisor
