In-home or risk-adjustment experience, Previous 1099 (PRN) experience, Bilingual is a plus
What You'll Do.
Conduct in-home wellness assessments
Conduct in-home risk-adjustment assessments
Perform focused histories
Perform focused exams
Perform medication reviews
Educate members on preventive care
Educate members on chronic disease management
Document visits in the EMR
