What They're Looking For.
Must Have
2+ years experience working with families/supports in healthcare, Lived experience as eating disorder support, Ability to connect with diverse people, Ability to identify and direct patients/families, Oral and written communication skills, Organizational and time management skills, Comfortable working remotely, Comfortable working with new technologies, Comfortable working on camera

Provide skills-based individual sessions

Guide caregivers to anticipate challenges
Guide caregivers in coping-ahead strategies
Maintain availability calendar
Schedule caregiver appointments
Conduct proactive outreach
Perform missed appointment recovery
Track caregiver engagement

Engage in problem-solving
Provide tangible solutions

About the role The Family Mentor is an integral member of a patient’s multidisciplinary care team. The role of the Family Mentor is to ensure that caregivers successfully integrate into Equip treatment by adhering to their care plans and fully engaging in treatment. This goal is achieved through proactively identifying and addressing barriers to treatment, identifying opportunities to personalize Equip’s programming to address family needs, and building caregiver skills.
