What They're Looking For.
Must Have
High School Diploma or GED, Previous IKC experience required, 2+ years professional experience in customer service, patient-facing, or telephonic outreach roles
Nice to Have
Associate's or Bachelor’s Degree preferred, Prior healthcare experience preferred
What You'll Do.
Facilitate CHE completion
Perform outbound calls to patients
Welcome new patients to the program
Collect new concerns from existing patients
Escalate concerns to RN or Nurse Practitioner
Maintain accurate documentation
Communicate patient updates to the care team
Demonstrate interpersonal skills to build trust
Influence patient participation
Overcome barriers to engagement
Organize outreach efforts by urgency
Coordinate care for patients
Ensure access to eligible resources/programs
Assist with referrals
Assist with provider appointments
Respond to inbound calls from patients
Respond to inbound calls from providers
Respond to inbound calls from Village resources
Complete non-clinical screenings
Complete non-clinical pathways
Escalate to Integrated Care Nurse
Assist clinical team with moving patients
Complete tasks as directed
Identify eligible resources for members
Collaborate with Integrated Care Nurses
Collaborate with dialysis clinics
Collaborate with health plans
Collaborate with other stakeholders
Help members enroll in programs
Communicate directly with patients
Maintain confidential documentation
