What You'll Do.
Facilitate CHE completion
Coordinate with Nurse Practitioners
Perform outbound calls to patients
Schedule telehealth CHEs
Escalate concerns to RN
Maintain accurate documentation
Communicate patient updates
Demonstrate interpersonal skills
Build trust with patients
Influence patient participation
Overcome barriers to engagement
Organize outreach efforts
Coordinate care for patients
Ensure access to resources
Assist with referrals
Assist with provider appointments
Respond to inbound calls
Complete non-clinical screenings
Escalate to Integrated Care Nurse
Complete tasks as directed
Identify eligible resources
Collaborate with stakeholders
Communicate directly with patients
Maintain confidential documentation
Demonstrate two-way communication
