Demonstrated experience in a role requiring detailed medical record review and collection, Proficiency with electronic medical record (EMR) systems, Exceptional organizational skills and attention to detail, communication skills, both written and verbal, for corresponding with patients, clinicians, external providers and colleagues, Ability to manage and track multiple tasks simultaneously, Ability to work independently and collaboratively, Working Knowledge of HIPAA regulations, Active Medical Assistant Certification, At least 1 year experience in a primary care, endocrinology, or urgent care setting

What You'll Do.

Supporting patients with determining program eligibility

Collecting all necessary records to confirm eligibility

Keeping patients informed throughout the entire process

Ensuring patients get started with the program and their care in a safe and streamlined way

Detailed review of patient records

Manage all work related to requesting

and tracking medical records from external Primary Care Providers (PCPs) and specialists

Ensuring HIPAA and other confidentiality/privacy compliance during the record collection process

All Form Health patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. About the Role: We are looking for an Eligibility Specialist who will be responsible for supporting patients with determining program eligibility, collecting all necessary records to confirm eligibility, and keeping patients informed throughout the entire process. This role will help us ensure patients get started with the program and their care in a safe and streamlined way. This will be a full-time opportunity, reporting to Vanessa Ramirez, Manager, Eligibility. This is a remote position located anywhere in the USA.
