What They're Looking For.
Must Have
High school diploma or equivalency, Minimum of three (3) years of relevant Medi-Cal/Medicaid enrollment experience, Experience working directly with the county DPSS submitting, managing, and appealing Medi-Cal enrollments, Demonstrate in-depth experience with complex Medi-Cal enrollment case management, Knowledge of Medi-Cal rules, processes, and practices
Nice to Have
Bilingual (Spanish/English) strongly preferred, interpersonal skills, comfortable speaking to seniors in a caring manner
What You'll Do.
Identify participants for Medi-Cal redetermination
Educate participants on forms completion
Assist participants with form completion
Conduct home visits for form completion
Obtain documentation for eligibility determination
Complete and file applications
Maintain follow-ups with participants
Maintain follow-ups with center staff
Maintain follow-ups with government agencies
Ensure timely processing of applications
Maintain working relationship with DPSS
Ensure compliance with DHCS policies
Ensure compliance with CMS policies
How You'll Work.
Team & Collaboration
Liaison between participants and WelbeHealth center team; Support the Eligibility & Reconciliation (E&R) team; Work with center staff
