Must Have
Master’s Degree in Social Work, Current licensure as a Licensed Certified Social Worker (LCSW) OR Licensed Independent Clinical Social Worker (LICSW), Effective verbal and written communication skills, Working knowledge of Word, Excel, Adobe, and Office365 including Outlook, OneDrive, and Teams, Ability to use computer-based technology including a computer/laptop and cell phone, Valid driver’s license and reliable transportation
Nice to Have
Two (2) years of pediatric experience
What You'll Do.
Demonstrate knowledge and understanding of all areas of infant-toddler development.
Utilize a parent coaching model in work with families.
Participate in screenings
and home visits as a member of a trans-disciplinary team.
Provide home-based services to families across the catchment area.
Collaborate with community providers to ensure continuity of services for families including child care providers
social service agencies
and specialty providers serving children with autism or hearing/vision loss.
Provide service coordination to assigned caseload including coordinating the development
and review of the Individual Family Service Plan.
