Must Have

Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire, 4-6 years in long term/skilled care or similar experience, visual acuity adequate to perform position duties, ability to communicate effectively with others, hear, understand and distinguish speech and other sounds

What You'll Do.

Providing care and services to meet the needs of the assigned facilities through strategic and accountable leadership.

implementation and adherence to policies and procedures.

Coordination of orientation programs for new staff.

Development and engagement of existing staff.

Leading quality improvement develops monitoring tools

uses data to drive improvements in care and services.

Total nursing care of residents and the accuracy of the nursing care records.

Selection of prospective residents based on nursing care needs and nursing personnel competencies available.

Overseeing all Federal/State SNF Conditions of Participation including admissions

MDS/Care planning process

restorative nursing and care delivery.

Establishing and implementing policies/procedures for the total care of residents inclusive of nursing activities

and delegated medical care.

along with the Administrator

the numbers and levels of nursing personnel.

Establishing staffing pattern to provide quality nursing care 24 hours/day.

Participating in planning

and budgeting for the facility.

Remaining aware of the safety needs for staff and developing policies relevant to quality of care.

Coordinating the QAPI program and facilitating staff participation in QAPI.

Delegating and monitoring non-nursing responsibilities to other staff consistent with their training

and legal authorization and with nursing home policy.

Understanding/coordinating Medicare/Medicaid coverage requirements.

Assuring that a comprehensive plan of care is established and implemented per regulations for each resident.

Interacting with the physicians/providers to plan care for the residents.

Assuring that discharge and transfer planning for residents is conducted.
