What They're Looking For.
Must Have
High School Diploma (Required), Drivers License (Valid) - USA, Experience working with adults with a mental health diagnosis and/or developmental disabilities | Not Required
What You'll Do.
Develop positive relationship with individuals
Assist individuals in achieving dreams and goals
Assess person receiving services
Provide supports that facilitate achievement
Ensure rights of person receiving services are protected
Serve as resource for person served
and monitor people receiving services
Maintain safe working environment
Educate people receiving services on procedures
Facilitate person-centered relationships
Implement person's plan fully
Complete all required documentation
Complete daily progress notes
Assist in maintaining records
Substitute in-house or in community
Assist new staff with orientation
Sleepover at residential setting
Attend and participate in meetings
Maintain certification in training requirements
Comply with agency policies and procedures
Follow service definition guidelines
Perform other duties as assigned
