What You'll Do.
Assess person receiving services for hopes
Provide supports that facilitate achievement
Support people receiving services in developing relationships
Assist people receiving services in participating fully in
Ensure rights of each person receiving services are
Promote rights of each person receiving services
Assist people in developing advocacy skills
Participate in advocacy efforts
Utilize personal advocates
Provide support to meet emotional needs
Provide support to meet physical needs
Provide support to meet medical needs
Facilitate positive relationships with people receiving services
Promote self-determination in all areas of life
Provide input into assessment processes
Provide recommendations into assessment processes
Provide input into planning processes
Provide recommendations into planning processes
Implement person's plan fully
Document as required by service definition
Document as required by agency policy
Complete all required documentation
Complete goal completion documentation
