Must Have
High School Diploma (Required), Drivers License (Valid) - USA
Nice to Have
Experience working with children or adults (dependent upon population served) with a mental health diagnosis, an intellectual/developmental disability, or substance use disorder
What You'll Do.
Assess person's hopes
Support people in developing relationships
Assist people in participating fully in community
Ensures rights of each person are protected
Provide support to meet emotional
Facilitate person-centered
positive relationships
Provide input into assessment and planning processes
Implement person's plan fully and document
Complete daily progress notes and communication log
Assist people with day-to-day operation of their home
Drive and travel as required
or provide transportation
Assist people with knowledge of emergency procedures
Utilize effective judgment when dealing with safety issues
Assist new staff and/or current staff with orientation
Sleepover at a residential setting may be required
Attend and actively participate in meetings and training
Demonstrate knowledge of and comply with all agency policies
Follow service definition guidelines for services
Complete all other relevant responsibilities as assigned
