What You'll Do.
Assess person's hopes
Provide supports that facilitate achievement
Support people in developing relationships
Assist people in participating in community
Ensure rights are protected and promoted
Assist people in developing advocacy skills
Provide support for emotional
Facilitate person-centered relationships
Promote self-determination in all areas
Provide input into assessment and planning
Implement person's plan fully
Accurately complete all required documentation
Complete daily progress notes
Complete communication log
Attend and participate in meetings
Attend and participate in training
Maintain certification in training requirements
Demonstrate knowledge of and comply with policies
Assist people with day-to-day home operation
Assist people during late night hours
Drive and travel as required
Arrange for transportation
Provide transportation to people
Assist people with knowledge of emergency procedures
Assist people with personal safety
Utilize effective judgment with safety issues
Conduct fire safety drills
Inspect safety equipment
Practice safety procedures in community
Assist new staff with orientation
Assist current staff with mentoring
Assist current staff with training
Follow service definition guidelines
