What You'll Do.

Lead or support clinical operations workflow development and iterative improvement
Support Quality Improvement/Population Health Managers with meeting preparation
data collection and analysis

Assist with implementing and monitoring workflow and/or screening fidelity
Oversee volunteers for outreach activities
Utilize internal and external reports to assess patient outreach needs and determine actions to close care gaps
Perform and document patient outreach efforts following NeighborHealth protocols
Maintain knowledge of MyChart and patient access workflows
Recognize “red flag” or urgent symptoms and determine when immediate nurse involvement is required
Provide EMR and technology training
Handle non–call-related functions such as email correspondence
Navigate EPIC to access relevant information and perform duties
Handling calls appropriately
Employing effective problem-solving skills
and canceling appointments
concise messages and routing them appropriately
Managing refill requests and encounters accurately and effectively according to guidelines
