What They're Looking For.

Must Have

Degree in Occupational Therapy, Active, unencumbered OT license, 2+ years of clinical experience, Reliable transportation, Valid driver’s license, Proficiency with technology, Digital documentation proficiency

Nice to Have

Experience in home health, Experience in aging in place, Experience in disability services, Comfort working with new systems, Home modification work experience

What You'll Do.

Conduct in-home assessments

Conduct functional evaluations

Deliver recommendations for home modifications

Educate clients on fall prevention

Educate clients on safe mobility

Engage clients in collaborative care planning

Engage families in collaborative care planning

Work closely with Director of Clinical Operations

Share findings with Director of Clinical Operations

Support process improvement initiatives

Maintain accurate documentation

Maintain timely documentation
