What They're Looking For.
Must Have
Bachelor's degree or equivalent experience, Experience working with intellectually disabled or brain injured adults, Ability to manage a client caseload, Valid driver's license, Reliable vehicle, Ability to work community based position
Nice to Have
Experience working with adults with developmental disabilities, Ability to advocate for community supports and services
What You'll Do.
Assist in completing assessments
Assist in completing Individual Service Plans
Ensure individuals are involved in goal development
Document daily progress notes
Develop and maintain professional working relationship with individuals
Develop and maintain professional working relationship with families
Develop and maintain professional working relationship with providers
Coordinate service delivery for cases
Advocate for the consumer with outside agencies
Advocate for the consumer with outside providers
Provide service linkages to other providers
Identify and schedule appointments for individuals
Schedule appointments with individuals
