What You'll Do.
Assist in completing assessments
Assist in completing Individual Service Plans
Ensure individuals are involved in goal development
Document daily progress notes
Develop and maintain professional working relationship with individuals

Coordinate service delivery for cases

This division offers support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships. The ideal candidate will have experience working with adults with developmental disabilities and will have the ability to advocate for community supports and services on behalf of the individuals. Responsibilities: Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards. Ensure individuals are involved in identifying and developing their own goals. Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements.
