Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience, Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience, Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience, Experience working with adults with a mental health diagnosis and/or substance use disorder, Valid Drivers License
What You'll Do.
Provide psychoeducation
Assist with behavioral and substance use disorder treatment
Assist with development of relapse prevention and disease
Coordinate initial and ongoing assessment activities
Develop and/or participate in initial development
Ensure PCP implementation
Consult with medical and non-medical providers
Engage community and natural supports
Include input in the person-centered planning process
Ensure linkage to clinically appropriate services
Arrange for psychological and psychiatric evaluations
Monitor and document beneficiary progress
Document effectiveness of strategies and interventions
Complete functional needs assessment(s)
Determine scope and anticipated outcomes
Communicate beneficiary progress to team leader
Communicate effectiveness of strategies to team leader
Provide intensive case management
Identify and provide emergency crisis services
Respond and coordinate other services
Participate in agency on-call structure
Facilitate debriefing session following crisis event
Make necessary changes to PCP or crisis plan
Linkage and referral to formal and informal supports
Assist with beneficiary housing search
Engage landlords to rent to beneficiaries
Write reasonable accommodation letters
Assist with connecting beneficiaries to financial and in-kind
Assist with rehousing beneficiaries
Assist in developing daily living skills
Ensure admission and discharge procedures are followed
Coordinate admissions and discharges
Monitor utilization of services
Ensure agency provides services consistent with authorization
Prepare reports for team leader
Communicate progress or lack of progress
Communicate involvement in treatment
Maintain timely and accurate service documentation
Complete and submit accurate billing information
Maintain comprehensive medical records
Maintain positive working relationships
Drive and travel extensively to community locations
Provide and/or arrange for transportation for people supported
Attend and actively participate in meetings
Attend and actively participate in training
Maintain certification in all agency
Demonstrate knowledge of and comply with all agency
Demonstrate knowledge of emergency procedures
Assist in crisis situations
Follow service definition guidelines
Complete all other relevant responsibilities as assigned

**Scheduled Weekly Hours:** 40 **Schedule** : Monday-Friday (8am - 5pm) Flexibility for rotating after hours on-call coverage. **Starting Pay:** $22.00 **Education We 're Looking for:** Minimum of a Bachelor Degree (dependent upon experience) (Required) **Certifications We 're Looking For:** Drivers License (Valid) - USA, Qualified Professional (QP) - Monarch-DSM **Experience We 're Looking For: ** Experience working with adults with a mental health diagnosis and/or substance use disorder | 1-4 years (Dependent Upon Education - see Job Description) | Required **Additional Details:** _**A Qualified Professional (10A NCAC 27G .0104) must have one of the following:**_ * Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience required * Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience required * Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience required **Job Description Summary:** The primary responsibilities of the Community Support Team Professional III are to provide clinically-based supports to individuals with a mental health or substance abuse issue to achieve their personal dreams and goals of restoring what they define as their natural function within their community. **What You 'll Do:** • Provides psychoeducation as indicated in the PCP. • Assists the Team Leader with behavioral and substance use disorder treatment interventions. • Assists with the development of relapse prevention and disease management strategies. • Coordinates the initial and ongoing assessment activities. • Develops and/ or participates in the initial development, implementation, and ongoing revision of the PCP, and ensures its implementation. • Consults with identified medical (i.e., primary care and psychiatric) and non-medical providers, engages community and natural supports, and includes their input in the person-centered planning process. • E
