What You'll Do.
Assess person's hopes
Provide supports for achievement
Ensure rights are protected
Assist in developing advocacy skills
Participate in advocacy efforts
Utilize personal advocates
Provide relief to caregiver
Serve as resource for community agencies
Supervise individuals receiving services
Educate individuals receiving services
Monitor individuals receiving services
Provide one-on-one support

Facilitate positive relationships
Promote self-determination
Provide input into assessment
Provide recommendations into planning
Develop individual's plan
Implement person's plan
Implement respite plan
Complete all required documentation
