What You'll Do.
Assess person's hopes
Provide supports that facilitate achievement
Protect and promote rights of person
Assist people in developing advocacy skills
Participate in advocacy efforts
Utilize personal advocates
Provide periodic relief to caregiver
Serve as resource for community agencies
Supervise individuals receiving services
Educate individuals receiving services
Monitor individuals receiving services
Provide one-on-one support
Meet emotional needs of person
Meet physical needs of person
Meet medical needs of person
Maintain safe environment
Practice safety procedures
Facilitate positive relationships
Promote self-determination in life
Provide input into assessment
Provide recommendations into planning
