Full Job Description

**Job Description:** _We 're committed to bringing passion and customer focus to the business._ The Community Outreach Representative is responsible for engaging Medicare seniors to create organic patient growth in the local communities. The Community Outreach Representative will work very closely with local community partners and clinics to make sure seniors have access to value-based preventive primary care. This role will report into the local Outreach Community Manager. * Deliver and coordinate educational presentations to existing and prospective patients. * Create and maintain working relationships with referral sources. * Attend lead-generating events in the community and at our clinics. * Engage potential members through phone, text, and email. * Manage and report on leads through CRM or other. * Health plan partner collaboration and coordination. * Develop and maintain local broker relationships. * Coordinate with marketing to promote events. * Conduct prospective patient tours in partnership with operations. * Develop relationships with other community partners, such as non-profits to acquire new patients. * Collaborate with the operations and clinic staff to coordinate services/events for prospective patients. * Collaborate with the broker and local health plan community to increase awareness and patient enrollment. * Collaborate with marketing to engage in social media and content development * Hybrid (50% field/home office) * Convert prospective patients to enrolled/active patients with booked visits and assigned PCP. **Qualifications** * Bachelor’s degree in business, sales, marketing or equivalent work experience * At least two years of experience working in the healthcare sector * At least one year with Medicare, Value-Based Care is a plus * Experience working in a quota-based comp model * Experience with PowerPoint and presentation skill * Experience with reporting and tracking through Excel. * Excellent written and verbal communication skills If yo
