Diploma in health or social service work field, 2-3 years related practical experience, Knowledge of community resources, Knowledge of social determinants of health, Experience with trauma-informed care, Experience with anti-racist practices, Experience with anti-discriminatory practices, Experience with anti-oppressive practices
Nice to Have
Registration with Ontario College of Social Workers, Understanding of acute care setting, Ability to navigate hospital systems

Facilitate supportive discussions

Provide system navigation supports
Support patients' social determinants of health
Assist with housing needs
Assist with transportation needs
Assist with employment needs
Assist with food needs
Assist with health insurance needs
Assist with government documents needs
Contribute to interdisciplinary care planning
Accompany patients to programs
Accompany patients to services
Support patients' post-discharge needs
Support patients' post-discharge goals
Provide education on substance use
Provide education on harm reduction
Provide education on community resources
Advocate for stigma reduction
Participate in program development
Participate in quality improvement initiatives
Liaise with external partners
Maintain up-to-date knowledge
