What You'll Do.
Provide care coordination
Navigate resources for clients
Conduct telephonic outreach
Follow up with clients
Coach clients on enrollment
Administer SDOH screening
Complete intake forms
Assist clients with prioritizing goals
Create client-centered care plans
Coordinate with community nonprofits
Provide resources to clients
Improve health literacy
Improve self-sufficiency
Assist with scheduling appointments
Complete applications
Maintain client privacy
Uphold confidentiality
Participate in team meetings
Commute to client homes
