Must Have
High school diploma or equivalent, Valid driver's license in Nevada, Use personal vehicle for client visits
Nice to Have
2-3 years direct care coordination experience, Experience with nonprofits or social service agencies, Additional language skills
What You'll Do.
Provide care coordination
Conduct outreach and coaching
Administer SDOH screening
Assist with goal prioritization
Create client-centered care plans
Coordinate with community resources
Provide resources for health literacy
Assist with case management tasks
Maintain client privacy
Participate in team meetings
Commute to client homes
