What They're Looking For.
Must Have
High school diploma or GED, Community Health Worker certification, 2,000+ hours CHW work, 1-3 years healthcare experience, 2-5 years community work, English and Spanish fluency
Nice to Have
Knowledge of Latino customs, Familiarity with Google workspace, Past EHR documentation experience, Training in motivational interviewing, Well-known in local Latino community, Well versed in local resources
What You'll Do.
Play active role in patient registration
Organize community engagement
Conduct outbound calls to patients
Conduct outreach to patients
Complete initial intake
Engage with assigned patients
Provide care navigation
Provide appointment logistics
Provide prescription drug support
Ensure care plan adherence
Assess for social determinants of health needs
Enroll patients in SDOH programs
Refer to community resources
Collaborate with multidisciplinary care team
Contribute to care plans
Solve complex patient needs
Document patient interactions
Document care team interactions
Participate in community events
Support patient activation
Build relationships with key contacts
Facilitate group education sessions
Liaise with community organizations

Community Health Worker/Promotor(a) de Salud at Zócalo Health Work from Home Danville, CA /or San Ramon, CA (Contra Costa County) (Full Time) Compensation: $29.00 - $31.00 per hour About Us Zócalo Health is the first tech-driven provider built specifically for Latinos, by Latinos.
