HEALTHCARE COMMUNITY CONNECT PROJECT DIRECTOR POSITION SUMMARY The Healthcare Community Connect Project Director provides strategic leadership, operational oversight, and stakeholder engagement for community-based healthcare initiatives designed to improve access to care, address social determinants of health (SDOH), enhance care coordination, and strengthen partnerships between healthcare providers, community organizations, government agencies, and other stakeholders. The Project Director is responsible for planning, implementing, monitoring, and evaluating Community Connect programs that improve health outcomes, reduce disparities, and support population health objectives. This role serves as the primary liaison among healthcare organizations, community partners, executive leadership, and funding agencies to ensure successful project delivery and sustainable impact. KEY RESPONSIBILITIES PROGRAM LEADERSHIP & STRATEGY - Develop and execute strategic plans for Community Connect initiatives aligned with organizational goals and community health needs. - Lead large-scale community health transformation projects from planning through implementation and evaluation. - Identify opportunities to improve care
