High School Diploma required, Two years of relevant experience (High School), One year of relevant experience (Associate's Degree)
Nice to Have

What You'll Do.
Provide case management
Provide health system navigation
Improve coordination and outcomes
Collaborate on treatment goals

Act as liaison to community agencies
Assist in advocating for client needs
Make referrals for services
Schedule appointments
Follow up with appointments
Plan for transportation
Coordinate with existing providers
Facilitate communication

Complete applications for community services
Problem solve issues interfering with care
Maintain positive relationships with providers
Maintain positive relationships with agencies
Assist team in making referrals
Support individuals with follow through
Coordinate clinical patient services
File encounter notes in EMR
Collect clinical data
Work cooperatively with team members
