What You'll Do.
Provide individual co-occurring sessions
Assist consumer with WRAP
Assist consumer with crisis planning
Support medication delivery
Collaborate with PACT team
Assess consumer concerns
Assess mental illness symptoms
Assess behavior in response to medication
Attend daily morning meetings
Provide crisis assessment
Provide crisis intervention
Complete and submit accurate reports
Complete chart documentation
Meets program obligations
Operates within program budget
Complete PACT Training

Interventions may be offered via individual and group modalities. Interventions take into account each consumer's stage of treatment and will assist consumers. As an integrated health organization Acenda values the use of evidence-based practice or EBP's. **Program Info** **Program for Assertive Community Treatment (PACT)** is an intensive, team-based behavioral health service that offers individualized support in the community to people with serious mental health conditions and high service needs. PACT assists individuals in their recovery from mental health conditions and in developing fulfilling lives, as well as increasing their time and engagement in community settings rather than institutions such as hospitals and jails. **Key Responsibilities** * Provide individual co-occurring sessions to consumers through symptom assessment, management, and supportive counseling, as well as assistance to the consumer regarding Wellness Recovery Action Plans (WRAP), an
