Serves as the medication expert on medical and surgical units and is available to provide time-sensitive, accurate information for other healthcare providers in acute scenarios; Provides pharmaceutical care, dosing and guideline recommendations to impact patient care and decrease readmissions; Provides guidance for drug selection and pharmacotherapy regimens in accordance with evidence-based guidelines

What They're Looking For.

Must Have

Doctor of Pharmacy degree, Completion of PGY-1 residency or 2 years of demonstrated clinical experience, Current Maryland State License

Nice to Have

Completion of PGY-2 residency or board certification in pharmacotherapy

What You'll Do.

Carrying out Clinical Pharmacy services

Promoting evidence-based pharmacotherapy and sound pharmaceutical care

Development of medication management protocols

and drug therapy policies

Serving as a Clinical Pharmacy resource for the health system

Conducting departmental activities to meet goals

objectives and policies of the pharmacy department

Participating in improvements in interdepartmental policies and procedures

Maintaining accurate records

Completing all core competencies and documenting clinical activities

Supporting the Investigational Drug Service

Serving as the medication expert on medical and surgical units

Providing pharmaceutical care

dosing and guideline recommendations

Providing guidance for drug selection and pharmacotherapy regimens

Completing pharmacotherapy drug class reviews

formulary addition/deletion

assimilation of evidence-based protocol/order-sets

medication safety related activities and/or targeted medication expense reductions

Assisting in appropriate transition of care services

Serving as the pharmacy representative on multidisciplinary committees

Developing pharmacotherapy-specific specific educational programs

standard operating procedures

Educating other healthcare providers on approved treatment guidelines and criteria

Serving as a resource for Pharmacy School Students and AAMC Pharmacy Residents

How You'll Work.
