American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice, Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required, Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required, Basic Life Support (BLS) - ARC American Red Cross 180 Days required, Pharmacist possessing PharmD, PGY2 Residency in Critical Care, Critical Care Pharmacist Board Certification (BCCCP)
What You'll Do.
Provide comprehensive clinical pharmacy services across ICU
and emergency department settings
participation in codes
rapid response events
and acute stroke response
Ensure safe and effective medication use through pharmacokinetic dosing
antimicrobial stewardship collaboration
and medication safety oversight within assigned patient care areas
Serve as a preceptor and mentor for PGY-1 and PGY-2 pharmacy residents
Participate in residency program activities
including medication use evaluations (MUEs)
and acute care skills training for resident onboarding
Contribute to PGY-2 critical care residency programming through precepting
and research mentorship
Inpatient pharmacy operational responsibilities included
Interprets and screens medication orders for appropriate agent
and potential drug interactions
Safely compounds and dispenses medications for administration to patients
orders appropriate laboratory tests
and interprets serum drug levels utilizing pharmacokinetic provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes
and monitor the costs associated with drug therapy
Provides appropriate supervision and direction within appropriate skill level to technicians
and other support personnel
Engages in continuous study of the entire professional field to maintain the professional competence
and skills necessary for satisfactory performance of all assigned responsibilities
Completes required continuous training and education
including department and Spectrum Health specific requirements on a timely basis
Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications
Ensures that services are provided in accordance with State and Federal regulations
